Please download the Edison Patient Forms (2 quick pages) below. This PDF includes the necessary Medical Release and Patient Authorization information. Our team will need you to complete and sign both pages so that we can get your journey with Edison started.
Once filled out and signged, please return the forms to Edison by scan & email, or fax. Once our team receives these forms, we then have your authorization to proceed and we will to connect you with a SmartCare Center to begin the medical review process.
